Reducing the burden of disease
Chronic disease management is an emerging discipline that aims to improve the health of a defined population with a chronic illness, such as:

- Respiratory disease
- Congestive heart failure
Chronic diseases make up more than 80 per cent of Australia’s overall disease burden due to death, disability and diminished quality of life (Department of Health and Ageing; A Mandate for Action on Chronic Disease Prevention, Unpublished draft document, 2004).
Chronic diseases also account for a significant number of potentially avoidable emergency department presentations, hospital admissions and readmissions.
